How to include text on email notification

I have created a disclosure form that includes a body and a few fields for user input.  I would like to have the entire form emailed to me when the user clicks submit.  So far I am only able to get the input data sent to me. 

That is because texts are not included on emails by default. I suggest to copy and paste the text in the email template instead so it will be included on email. You can also customize your email template if you want.
