Forms not appearing

Our "UG Part-time Single-Unit registration form" does not appear to be loading correctly. The back-end (form builder) still works fine, but it no longer loads up correctly when trying to use the front-end, and all you get is a blank page. The form link is supposedly this: https://form.jotformeu.com/51903326313346 which we have been using for many months now; it must have only stopped working in the last week or two.

Can this be fixed as a matter of urgency please?

This appears to be the second issue we have had since transferring data to the EU servers - might there be a problem there?
