Form Calculation: Calculate field value does not work

It worked earlier today, but after the calculation wizard layout  changed it does not work

If Insect Infestation is equal to select

insert (defect2_0_3)into Score

Defect7 and Score is a matrix

I've checked your conditions. I see that, you have 146 conditions. =)

I think, the problem is on  Insect Infestation filed.

Let me try to figure out the cause of this issue. I will get back to you on this soon.
