We're consistently getting feedback that our form has technical issues

I'm not sure what the issue is with this form.  It works sometimes...other times it does not.  It works well on some devices...others, not so much.  Can you help me diagnose the root cause?  

I also see there is a random "back" button under the submit button on the live form....the form builder shows nothing in that space.  Please advise as soon as possible.
