    Its very difficult for us to offer specific advice without seeing the source code of your webpage.  

    There is a multitude of different HTML/CSS variations that could be used to have your webpage looking as it does, each of those options would affect the CSS of a div we would use to position the form where you want it. 

    Are you even able to share the pages source code somewhere? 

    e.g by pasting it to http://pastie.org/ and sharing us the link it generates?
