jot form header in emails - #1

What is the setting that allows for the jot form header to appear in auto emails? It does not most forms but not on others and I can't figure out what it is. screen shot #1 attached.

Thank you for contacting us.

You can see the URL or change the logo by following these steps.

Click on the email icon at the top toolbar.

Click Edit on the email notification that you want to edit.

Now, just click on the image, and then click on the image properties icon, there you will see the URL to the image, you may also change it, if the current image is not being displayed properly, you may use this URL instead : http://www.jotform.com/images/win2_title_logo.gif?new
