Form not loading, one out of four times the page is loaded.

    This is the second time i use jotform. The first time alll went well, but in this form i'm getting this issue. 

    The form itself does not load. If you refresh the page several times, you can see that 20% of the times, it fails to inject. 

    Can someone tell me what goin on ? Is it a problem with your web application ?

    Apparently, embeb - source code version does not fail to load the form ever. 
    Switched to that and the problem is fixed.
