Unable to submit form

Hey djoost ,did you every get your problem fixed? I am currently having a problem with my Submit button.  Please let me know.

I have tested your form that you last modified http://www.jotform.us/form/52934853099165. I was able to submit that form without any issues. When I submitted that form I got redirected to Thank You page.

If you have issues with some other form, please provide us ID of that form. If you have embedded your form to website then provide us URL of your website so that we can test your form there.

Also please explain in more details what kind of problem you have with submit button on your form.
