Unable to create a "pdf form"

    When I hit the "create PDF form I get this message

    This XML file does not appear to have any style information associated with it. The document tree is shown below.

    I cloned your form and I was able to create a PDF form without an issue.

    Please try clearing your browser cache and your form cache, then try creating a pdf form again.

    I get the same "Access Denied" error in Chrome and Firefox, after clearing both the forms cache and the browser cache. Please advise.
