    I am not quite sure if I understand if this thread is a separate issue from the original one you have in this link http://www.jotform.com/answers/754792.

    However, I presume that you wanted to have this text:

    I acknowledge that I :

        (1) The Document on Suitable Boats

        (2) The Important Notes referring to Kites & Wires Courses.

    In your form, the "Short Scrollable" widget seems to work but it is not mobile responsive as you have mentioned, is that correct? Could you please try this cloned form: https://form.jotform.com/60254792479971. See if the widget there is mobile responsive.

    In my case, this is what I did:

    1. First, I'll get the widget ID of the "Short Scrollable" widget. Here's how:

    1.1. Right click on the widget in the live preview of the form

    1.2. Choose "Inspect"

    1.3. It will open a browser console either at the bottom or right side page, in my case it is set at the bottom part. Make sure you are on the "Element" tab.

    1.4. Find the widget ID in element wrapped in <iframe> tag. In my case it's customFieldFrame_31

    1. How can i format it so that the Statements I Acknowledge that i have read...... are aligned to the left of the tabulated items (1) & (2) Image 50

    2. Note your widget ID, go back to the form builder, and set the widget's width to "0".

    1. How can i format it so that the Statements I Acknowledge that i have read...... are aligned to the left of the tabulated items (1) & (2) Image 61

    3. Open your form Designer and we will inject a custom CSS code.

    Here's mine:

    #customFieldFrame_31 {

    width: 100% !important;

    }

    The bold text is your widget ID, make sure to append a "#" at the beginning of it. Paste the custom CSS code in your "CSS" tab.

    1. How can i format it so that the Statements I Acknowledge that i have read...... are aligned to the left of the tabulated items (1) & (2) Image 72

    What it does is it will change your widget's width matching the screen display as to where the form is being viewed, making it mobile responsive. Here's a screencast:

    1. How can i format it so that the Statements I Acknowledge that i have read...... are aligned to the left of the tabulated items (1) & (2) Image 83

    Let us know if that works or if I misunderstood your concern.

    As an alternative to what my colleague suggested, if you still wish to use the checkbox fields:

    The "I acknowledge" text is aligned with the 1) and 2) lines, but if you would you like the 1) and 2) lines to be under the checkbox square instead, we can provide the necessary css code

    1. How can i format it so that the Statements I Acknowledge that i have read...... are aligned to the left of the tabulated items (1) & (2) Image 20

    We'll wait for more details to proceed.

    This response is in connection with the response from Charlie above, timed at 1:41pm on the 26th

    TO be honest i am totally confused, possible like yourself in regard to this issue, i started with what appeared to be a simple request, which on the face of it was going to be quickly answered by the thought of the widget doing the work, unfortunately, this was not to be the case.

    Next i get a response from another one of your colleagues who suggests another option of the Terms of Use idea...and the original thread title was changed to suit the Terms of Use concept, but i am not looking for Terms of use type solution, but i tired to understand what your colleague was referring to and i failed to get the suggested CSS solution to do wha i needed and i advised accordingly, and in so doing, i asked two more questions of clarification about the suggested solution she was recommending....

    as a result she opened two new threads which in my opinion on their own are difficult to understand. however that is my opinion. SO now i have 3 maybe 4 threads   coming out of one straight forward question and i still do not have a solution.

    However, i have read your thread on the work around for the Widget and will investigate futher.

    Thank You

    John

    Seems  like there are many suggestions on this. I apologize for all the confusion, once you finish editing your form, and you want to accomplish a customization to it, please do the following:

    1) Start fresh with a new thread.

    2) Let us know what is your form.

    2) Specify what you want to accomplish.

    So, we will check it and provide you the solution.

    Hello,

    Not sure what happened there as i do not respond via email i always use the forum web pages.

    Anyway, i think the missing response was to say that based on the recommendation by BDAVID, i did start a new thread at :-

    http://www.jotform.com/answers/756948-How-to-configure-Short-Scrollable-widget-to-be-responsive-with-control-over-font-etc-#0

    However i have since been informed by Kiran  that the Short Scrollable widget will not supply enough information in the Autoresponder email. please see    the following link for that response.

    http://www.jotform.com/answers/756151-What-information-will-be-included-in-the-email-notification-as-proof-of-acknowledgement#5

    I think i will revert to the solution that i have working , it is not ideal but it does what i want. thanks you everyone for trying to assist.
