Form not loading on WP website

Also,
we are not receiving the test emails. It used to be , that we had to set "From" to "no-reply ....."
when I was testing yesterday, it seemed to work when no selections were made in the setup for notices...???

I checked the embedded form http://www.jotform.us/form/60296800077153 on your website http://www.muddycreektackle.com/#map and I can see it working at this time.

I used Firefox and Chrome browser to check. Can you please check again on your side.
