I need set up help

I have watched all of the intro videos but I'm not sure how to move forward. I want to add the feature that will allow users to screenshot my site and provide feedback that is then emailed to me. I'm not sure which code under "Publish" I should be giving to my developer to make this happen. I can't find answers in any of the documentation on your site.

You can get the code, you should copy and paste it in your website, also you can replace the ID generated by default with the form that you want to add with the feedback button:

I forgot to mention that when you use the app to get a feedback, it creates automatically the form, once you finish all the steps, you will find the created form on My Forms page.
