Form is showing blank

Mine is the same tonight. White screen. Was working a couple of days ago. Client just emailed me with the issue.

I checked your form and it is displaying correctly on my end.  Is this still still not working on yours?

It seems good again now.

I wonder if it was to do with BT issues? A client pointed it out and it was blank until about 10 mins ago.

It's back now - so fingers crossed??

We had an issue with some forms displaying as blank earlier from certain domains.  It has since been fixed and forms should be available from all domains.
