Problem when embedding source code to website

Dear Christian / Jot Form support team, 

I would like to embed the form previously mentioned on this thread using the source code method as suggested by Chriistian.

After clicking on the COPY CODE button (in the publish section) and pasting the code on the html page section of our website the form rendering is incomplete, broken up without the background image. I am not a developer so I suspect I'm doing something wrong. I following on the How To Download link provided by Chriistian it seams that I'm following the right steps. 

What else do I need to do in addition to the above mentioned pasting to get the tom to work like when using all the other methods like the embed link or Iframe code?
