My lightbox form pops up as totally blank

No changes made to website but now when you click the form button, the lightbox pops up but its all white with no form.  Same issue with 3 other forms.  The button is the red "Pricing request" in the top right corner.

It looks like that your forms were disabled due to Expiration Date limit set on them.

You will need to clear these limits via form Preferences.

Then, re-enable the forms.
