How to recover deleted form?

is there a way to recover a deleted form that i created

Once a form is deleted from My Forms folder, it will be moved to Trash Folder. It is possible to restore form from Trash folder. Inside of this guide you can find how to do it:

However if form is Purged, it is not possible to recover that form.

I have checked your account and you have three forms, two in My Forms folder and one in Archive folder. I did not found any form inside of Trash folder. If you Purged form from Trash folder, then it is not possible to restore it.
