How can I format my pdf version of form

The PDF version of your form is not customizable - when it is created, it will try to copy the appearance of your online JotForm form as much as it can. It does not, however, allow for further customization.

If you need your PDF form to appear differently than your online form, you would have to change the appearance of your web form to how you want a PDF form to appear. After generating the PDF version of your form, you could then undo the appearance change made on the online/web form by using the form revision history.
