    Yes, you simply add the upload field to your form, and when you create the report, it will be included:

    Hi,
    1. Do you have a widget that generates a customer case ID number like in a help request?

    2. Do you have a widget that has a hidden field for product identification? Here’s an example. A product is out of stock, the form ask for an email address to notify the customer when it is in stock.

    3. Also, can you suggest an application like the one you use for posting help?
