1) Sometimes registration forms are sent to dropbox with registrant's name on a folder and registration is within folder (first item in screenshot).
2) Sometimes registration forms are sent as a pdf to dropbox, no folder (items 2-4 in screenshot).
3) More recently, items no longer are sent to dropbox. Help!

I cloned your form and made some tests with the dropbox integration. I can't seem to recreate the issue you are having. All the test submissions are sent to dropbox in their own folders.
Please try removing the integration, clearing your browser cache, and then reintegrate the form to dropbox again.
