Opening imported form from Google Docs

I just imported a document from googledocs.  Every time I try to view it I'm asked for my user name and password.  Nothing happens after I type the informaiton in.

It is a login form / sign in form, and as such it is against our terms of use:

Collecting Sensitive Information. The login form that was imported from your Google Docs has been removed from your JotForm account for that reason. If you wanted to create any other kind of form, please use our easy Form Builder to build your desired form to how you like it, as long as it is within our terms:
