I just imported a document from googledocs.  Every time I try to view it I'm asked for my user name and password.  Nothing happens after I type the informaiton in.

I assume this is about your form named "[Imported Form] Google Docs - create and edit documents online, for free." which could be found here:

    https://form.jotform.us/form/60408236585155

It is a login form / sign in form, and as such it is against our terms of use:

Collecting Sensitive Information. You may not use the JotForm to collect certain types of sensitive information, including but not limited to credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information.

The login form that was imported from your Google Docs has been removed from your JotForm account for that reason. If you wanted to create any other kind of form, please use our easy Form Builder to build your desired form to how you like it, as long as it is within our terms:
