    @ Amiedy

Is the form embedded on a website?

Can you share to us the URL of your website where we can test the form using mobile browser.

We will wait for your updated response.

You may also try first re-embed process. That is, re-embed the form on your website using instead its iframe embed code.

    user guide: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code

    then check/test again.

Let us know how everything goes.
