Why is the form no longer visible on my website?

    This is my contact is page. The form is no longer visible.

    Your form is disabled which is why it is showing unavailable. Please follow the steps below in order to enable it.

    1. In My Forms, select the form.

    2. Click the "More" button and then select "Enable" in the drop down.
