Autoresponder: Missing data in some fields.

how should the form fields be named?  i'm having the same problem

I did check your form and it seems properly, the only difference that I see is with the name, you are separating the Full Name field and adding it as two different fields, I'm not sure if that could work. However, do let us know which fields are you not receiving. All of them seem to be good.

By the way, the fields should be named as they are currently, they should not have spaces between, I did check and all of them look like do not have spaces in their names.
