Can you please help me integrate Mailchimp with Jotform?

I put in my API key but it won't let me add in the additional information needed to do the integration.

I tried checking the form that you might have used to integrate, but as it turned out, you do not have form yet under your account/username letsgethealthy  

Can you share to us the URL of the form you were trying to integrate to MailChimp?

We will check further if there is problem with the form.

Btw, I have also test it on my end, and the MailChimp integration was working on my test form.
