Form is Disabled

    Dear JotForm Support,

    My form (https://form.jotform.com/60526780488162) is currently disabled. I was wondering what it would take to re-enable it or if it is a problem on my end.

    I checked your forms and they look to be within our terms of use.  There was enough key words in one of your forms to set off our phishing detector.  The suspended your account.  I whitelisted your forms and you should no longer have any trouble with your current forms.  I also activated your account and the forms should be usable once again.
