If the URL part of the code embedded in your website begins with http://form.jotform.ca..., try changing it to http://www.jotform.ca... and let us know if it helps.
Initially, when you stated that "click to edit" is appearing, I thought that you simply had not changed the default labels of the fields. However, after checking the form and seeing that the labels were indeed changed, I now think it may be a latency issue with the edge server of the CDN (Content Delivery Network) nearest to your location. Forms at the http://form.jotform URLs are served via a CDN while those with the http://www... version URLs are distributed in the traditional manner.
