I tested your form using Static HTML app Method and it works correctly. I dont see any errors of your form. Can you re-embed your form source code using this:

Upon checking you facebook page were you embeded your form, I found out that your form is working properly. If you don't see your form, please logout to your page, delete browsing cache and cookies then log back in.

i can see that too my friend. the problem starts when i push the log in buton of the faceebok in the next page of my form (thing of vazhdoni button as next button). when i try to log in with facebook in order to proceed with sending of form it gives me

Given URL is not allowed by the Application configuration.

instead of log in options or further actions. that's my problem not just framing the form

We had another user reporting the same issue usually this is caused by incorrect domain set in your app settings. The site url set in your facebook should also be the same with the url provided by JotForm in your embed code. So if the form embed code is http://www.jotformeu.com/form/2060349598936  then please use use the following url in your website settings for the app in facebook:

Site URL: http://jotformeu.com
Site Domain: jotformeu.com
