I have checked your website and noticed that your form is showing fine, here is a screenshot when I tested your form:

If the issue still persists I would suggest you to check the style of your website, since there may be some CSS style that may be hiding the element on certain screen size.
Please test your website again on your end and let us know if the form is still not loading on mobile devices on your end.

Hey Kevin,
Thanks for the response. I saw that using an i-frame was the best route. When I select "platform" the javascript was shown as the best code. I saw somewhere in the forum to use i-frame. Im all set. Thanks again.

That's great to know. The iFrame embed method is recommended for Muse websites. Our Javascript embed codes will sometimes create a conflict in Muse and that's what causing your problem.
