Unable to create form

I have created a from and shared it to my one free user.

every time i hit create form it crashes!!!

Do you use a Create Form button under your 'Unispan' account? Could you reproduce this issue in different browsers?

What exactly is crashing on your side? Is this crashing the browser tab, or is there an error? Any additional information would be appreciated.
