If you meant to say, put map into your form then to your website.
Then, it seems the easiest way for you to this is to use the image field in power tools in JotForm builder. It has a step by step wizard and will guide you in putting your map image file into the form.Afterwards, you can then put the form in your website using this Share Forms guide.
