    You can achieve all these using widgets. For you to be able to have a Terms and Conditions section that shows the actual content of the TOS, you may use our Short Scrollable Terms widget.

    Now, I noticed you have added an Adobe e-Sign widget on your form. If you wish to use an alternative signature (a simpler one), you may try the Smooth Signature widget.

    And to allow your form to process payments as well, there are plenty of options. I highly recommend you go over this guide first so you have an idea of the basics [GUIDE: Setting-Up-Your-First-Order-Form] And here's the complete list of all the payment processors to choose from.
