Once you submit your form, you must follow through with setting up a Membership Inquiry Appointment with day and time of your choosing. You will be directed to the appointment site once you have submitted this form. Only "Face-2-Face" Appointments can be made if you are in the direct area of one of our Temples. If you fail to follow up with making an appointment you will not be contacted.
