hereby allow the Neubauer Hyperbaric Center to video/photograph my family member and/or me at the Neubauer hyperbaric Neurologic Center and assign all rights to the Neubauer Hyperbaric Center. The use of this video/photograph will be used for but not limited to educational, promotional purposes, broadcast television or for licensed third parties. I understand there will be no compensation for this assignment of all rights.
