KSU Student Media has a limited number of Digital Video Kits available for KSU student use.

Each Digital Video Kit contains:

One (1) KODAK Zi8 Pocket HD Video Camera

A Flash Memory Card / 16 GB

A Sony ECM-DS70P Microphone

Digital video cameras may be checked out for campus and off-site use for a  maximum of 3 days  per request.

We will do our best to meet your request, but please submit your request at least three (3) days in advance if possible. Reservations may not be submitted more than 60 days in advance.

If a Kit is available for your requested pickup date, you will receive a confirmation of the reservation date at the email you provide. Pick up the kit on the approved pickup date from Room 274 in the Student Center. Pickup times are M-F between 9 AM and 4:40 PM. Be prepared to show your KSU Student ID  and sign this form.

Instructions for the video camera in PDF format are located here: http://www.ipdop.org/pota/Kodak_Zi8_Manual.pdf . It is your responsibility to be familiar with camera use.

The entire Kit must be returned to Student Life (Student Center 274) no later than the date listed on the signed request form by 4 p.m. Requests to extend the return time may be accommodated if the Kit is available and has not been previously reserved. Call 770-499-3083 for return extension inquiries.

The recipient will bear responsibility for the on-time return of all equipment in the same condition as at the time of release. Digital cameras must be returned with all digital content removed from the hard drive/memory card. Individuals will be held responsible for any damaged, stolen or missing equipment.

PLEASE READ:

The above named individual will be responsible for damage, loss or theft and will provide due care and security for the above-described equipment until the equipment is returned to the University. If the equipment is stolen, a theft report must be filed with the KSU Department of Public Safety. Loss or theft  does not  excuse the recipient of liability. The recipient will bear responsibility for the return of this equipment in the same condition as at the time of release.

Video Kit cost of replacement is as follows:  KODAK Zi8 Pocket HD Video Camera ($180); Flash Memory Card / 16 GB ($60); Rechargeable Batteries ($19); Sony ECM-DS70P Microphone ($45).

A copy of this information will be printed, where it will be signed at the time of pickup and filed.

> It is understood that it is my responsibility to fund any repairs or replacement  should damage occur while equipment is checked out in my name. I will return this equipment at the time and date specified above unless prior arrangements are made.

> It is understood that a Registration Hold  may be placed on my registration in order to satisfy an obligation owed to the university. Failure to return equipment or fund repairs or replacements should damage occur while equipment is checked out in my name will result in a hold. Registration, transcript requests and graduation cannot proceed unless a hold is removed.
