  • By submitting this form You, named above, a prowd D411 sponsor, agrees and understands
    monthly invoices are sent     on or about the 25th of each month or 1/4 Year, 1/2 Year, or Year
    depending on which sponsorship package     you choose. Invoices are due when received.
    Your D411 invoice must be paid a timely manor     by credit card or debit card online
    or D411 can keep your information on file and process manually. 
    Sponsor understands that a late fee might be occurred if invoice in not paid on time. 

    Your sales represenitive can "come by" and get a check for an additional $25.00 each pick up.
    No checks can be accepted through the mail.

    An additional 10%-20% can be saved by paying semi-annually or annually
