Please fill out every portion of this form. If you are signing up for a non-fishing event please select "N/A" for the appropriate sections that are required for fishing events. Make sure that you provide accurate contact information as you will be contacted by an event coordinator. There will be 3 attempts to contact you. If we do not get a response from you then you will be dropped from the event list without further notice. If you need to cancel your spot for an event please contact us as soon as possible so that we may find a replacement to ensure that all of our Volunteers get to participate with a soldier. Some Volunteers drive many hours and travel vast distances to show their support, please be responsible and let us know if you are going to be able to make it. For SINGLE Soldier events you are not allowed to bring your family unless prior arrangements have been made. Please contact your Event Coordinator to answer any questions you may have. We look forward to providing you with a free trip of some sort with something that you may not have been able to do on your own. You will be asked periodically to take some pictures or video for our sponsors. You agree, by signing up for the event, to have your picture and/or your name listed in any media outlet we choose. Sometimes we will also have media attention for the event - newspaper, news channel, magazines, telephone - YOU DO NOT have to talk with the media if you do not want to. Every little bit of coverage helps and it works best coming from you - the soldier. Please keep us in mind when talking with the media and help us promote what we are doing.

In case of an Emergency situation and someone must be notified, please fill out the information below. Please give us the information to someone who is not attending the event with you. If you are part of a group that is coming and one person is in charge, please list their info here.

In this area you will be asked a series of questions regarding the event you are signing up for. Please know some information about the event and answer the questions accordingly. Failure to fill out the form completely and accurately may result in your entry form being deleted without notice.

Most of the events that we have listed are for soldiers to be able to go fishing. In the event that your family is invited to the event please understand that they may not be able to go fishing with you out on the boat. Special arrangements can be made to get you out on the water with your family. In most cases we have a boat big enough for the entire family. If you have a small child you may need to bring a lifejacket to make sure they have one otherwise they will not be permitted to go out on the boats. All other lifejackets can be provided except the small ones. All soldiers that sign up and participate in an event must go fishing and participate in any other sponsor related activities. The family may be invited to these activities but it is not required for them to attend. Every effort will be made to make sure that everyone in your family is taken care of and having a good time. We always try to make any lodging arrangements free for the soldiers and their families. If lodging is available, there will be a one family per room policy. Additional rooms that are needed are the responsibility of the soldiers. There also may be times when soldiers may need to pay a discounted price for food. All of these situations will be identified and you should already know before signing up for the event.

You agree to be bound by the Take a Soldier Fishing Hold Harmless Agreement. A copy of the HHA can be obtained from your Event Coordinator or directly from the TASF website at www.takeasoldierfishing.org - Take a Soldier Fishing will not sell, trade or otherwise give away any information that you provide on this form.
