PAST EXPERIENCE WITH CCP
What is your past experience with CCP? *	I submitted an application but was not placed on a team. I have previously participated on a CCP team. I applied and was invited to participate but had to decline. I do not have any experience with CCP.
PRIOR CONSULTING/ORGANIZATION CHANGE EXPERIENCE
Which of the following level of organization consulting/organization change experience do you possess? (This helps us determine diverse skill levels. Your answers will NOT exclude you from CCP): *	I have never done this kind of work. I have done simulations, role plays, case studies, college courses workshops, etc. about organization work. I have worked with others on change projects that included an experienced lead consultant or faculty person. I have worked for a few clients by myself where I had responsibility for the work but where another person had ultimate responsibility to the client. I have worked as an independent consultant with responsibility for the entire process.
How flexible is your schedule? (Check one) *	Completely flexible- my commitments allow me to meet almost any evening. Moderately flexible- I have 3-hour time blocks available to schedule meetings during several days or evenings. Somewhat flexible- I have 3-hour blocks available for meeting at least one day and one evening a week. Not very flexible, but I would like to see if I could make it work.
