MySource Testimonial Contest Entry Form
Entering is easy – but please read this information carefully before you enter.  Have a close look at the rules (we’ll wait), and then fill in the fields below.  Only audio files are eligible for entry, and attaching audio works just like an email attachment.  Enter as many testimonials as you like, but please complete the entry form for each piece of audio you submit.
This is important – any testimonial you enter must be broadcast on your local air prior to June 30, 2008, AND end with the line “My name is (John Doe), and (Station Name) is my source for (X).
All entries must be either :29 or :59 seconds long and be encoded at 128k mp3 files. Be sure to note and provide links to any multiplatform use of your entry in the appropriate fields. All entries must be received by 6pm EST on June 20, 2008. Don’t wait for the last minute – the web is least reliable when you have a deadline.
