In consideration of the material and services performed on myself. I hereby assume all risks of personal injuries and all loss of or damaged property from all causes and release Pageant P.R.E.P. 101, the persons performing the services, jointly and severally from liability. I hereby declare that I am over the age of 18 year’s (Parent or Guardian Signature requirement if under 18 year’s of age) I hereby give consent to Pageant P.R.E.P. 101 rights to use my Photographs for any educational, promotional and advertising purposes they see fit. Any promotional material remains the property of Pageant P.R.E.P. 101. We acknowledge and respect the privacy of individuals and this information is voluntary, but if not provided Pageant P.R.E.P. 101 may be unable to proceed with your service.
