NOTE: Your resume and picture will only be used internally by MMICC organizers at McGill. Resumes will also be compiled for the judging panel. Otherwise, we will never publish, copy or distribute your resume or picture to the public.

Please read the following statements and acknowledge by checking the boxes.

[A]. I am aware that I may be contacted by a McGill Ambassador or any member of the Executive Committee prior to the competition for logistical purposes.

[B]. I authorize the Executive Committee to disclose my name to the Marriott Residence Inn Downtown Montreal for my hotel reservation, and to also display my name, as well as any photos or videos taken during the event that include me, on the MMICC website or publication.

[C]. I am aware that my team is responsible for booking the necessary hotel rooms. I understand that I am responsible for checking-in no later than 3pm on March 23rd, 2010, and checking-out no later than 12pm on March 28th, 2010. I am responsible to inform the Executive Committee if I will be arriving early or extending my stay. I am also aware that my team will be divided in rooms based on gender, and that each delegation is given 1 advisor room (1 King bed) and 2 student rooms (2 Queen beds each).

[D]. I am aware that the Marriott will ask for 1 credit card per room as a guarantee during my stay. I understand that my team is responsible for paying all outstanding balances (if applicable) for accommodation before check-in and to settle for hotel costs upon check-out, including incidental charges, if applicable (e.g. long-distance calls, amenity fees, dry cleaning, etc.).

[E]. I am aware that the Marriott will book rooms on predetermined floors, and that MMICC does not allow room changes. During the competition, I understand that 1 of the 2 student rooms will be designated as the "Case Period Room" (where my team & I will work) on March 26, and that the choice will be made by the Executive Committee only.

[F]. I am responsible to provide a passport and/or visa and/or student ID card and/or proof of age to Canadian Customs, the Marriott Residence Inn, restaurants & venues, or any other government authority if requested. MMICC and McGill University will not be held responsible for any travel or legal complications which may arise prior or during my stay. It is my responsibility to request a "Letter of Invitation for Visa Request" from the MMICC Executive Committee if such is necessary for my travels.

[G]. I am aware that the legal drinking age in the Province of Quebec is 18 years old. I understand that, even if I'm more than 18 years old, I may be asked to show a proof of age (photo ID) in some restaurants or venues during MMICC.

[H]. As a participant, I agree to follow the MMICC Rules & Regulations, which will be effective throughout the competition: from 9am Monday, March 23, 2010, to 6pm to Friday, March 27, - 6:00pm. I am aware that these rules and regulations will be explained in detail to my team during the Rules Briefing. I acknowledge that not following these rules may result in disqualification from the competition.
