Form Fields

JotForm has pretty much any type of form fields you might need. Take a look at our 40 unique form fields and get a brief description of what each one of them do.

Heading:


   Add headings to indicate what the section below is about




Text Box:


Text inputs to add single line of text




Text Area:


     When you need a longer text entry








Drop Down:


Let users select one of the options



Radio Button:


Allows user to choose only one option





Check Box:


Allows multiple selections





File Upload:


   Let users send you photos or any other kind of files



Submit Button:


Users click on buttons to submit a completed form




Full Name:

   Makes sure user enters his/her name in    a specified format





E-mail:

Helps users to enter their e-mail correctly



Address:

   Allows users to enter their address in a correct format







Number:

Helps users to enter a number correctly



Phone:
Helps users to enter a phone number in a correct format


Birth Date Picker:

   Helps users to pick any date easily






Star Rating:

When you want to receive ratings, as stars, lightbulbs etc



Scale Rating:

   Allows user to give points based on a scale




Grading:


Allows user to select a total value into options as a grade







Slider:


   Allows user to visually select a number within a range




Range:


Allow user to define a number range



Spinner:


Makes it easier to submit numbers



Matrix:


   Allows users to select multiple values for multiple options








Form Collapse:


   Hides a section of the form. Split your form into expandable parts




Page Break:


   Splits your form into pages. Works best with a heading





Free Text (HTML):

Use this to format your text and  insert HTML Code.





Date Time:

  
 Ask date and time values in    your form




Password Box


Use this to ask for passwords in your form


Hidden Box


Hidden field is not seen but submitted with the form nonetheless



Captcha

   Captcha prevents spam submissions






Image


Add an image for your form. ie. Your company logo





Auto Complete

   Helps user to pick the exact value









Payments:

Allows you to collect payments through various international payment services

When a form is opened for editing, the "Setup and Share" tab has an icon for "Preferences" ("Update form setting"). When opened, these settings include "Unique Submission" with three choices: No Check, Loose Check, and Strict Check. I can find nothing that describes either the purpose of this setting or the meanings of the three choices. Please elaborate, including adding ALL of these form setting (aka Preferences) to your Screenshot List of All (!) Form Fields.

No Check: People can enter as many submissions as they would like.

Loose Check: Cookie based check. When a user submits this form the next time she comes to the form if the cookie is still on the browser she will not be allowed to post again.

Strict Check: Cookie AND IP address check. In addition to the cookie check if the user's IP address was used previous she will not be allowed on this form.

Is there a way to set the length of the cookie and IP. I want my users to use the forms once a month. Is there a way to reset the cookie or set a time period for the cookie?

No we don't have that feauture. What you can do is write a warning and ignore the second post in a month.

Also that is not the place for asking questions. You should have asked it in forum.

This is my new project since all updatees. I thought there was a way to make multiple selections from a drop list in the old version. Can that still be done? I have a very long list of choices and it's not practicle to list the down the page. If this feature is not available, can you suggest an alternative way for me to handle this?

Hi,
Can I duplicate fields in a form for user input, For example, if they have five trucks. They can click on a link that says add truck and the same fields show up to complete their entries for the new truck.

The categories are not really intuitive. Why is one thing a "form tool" and another a "power tool"?

There should be an option to list all objects in one alphabetical listing.

Is there a way of preventing word wrap on instruction or question fields so that they don't lengthen the overall form: ie.

I like how the Zot programmers are using a generic Captcha. I have to say that Re-Captcha is perhaps the worst (mini) form. For one, I do not want to link my Website back to Google. And, digitizing books does not make me feel all warm inside, either. Second, the image is extremely difficult to read and can be confounding. For example, one Re-Captcha image popped up with a pi (π) symbol! That is not an easy symbol to enter since it is not readily available on the keyboard (unless one knows how to access Unicode/extended ASCII character). So, thank you programmers for being smart!
