Sacrament Certificate Form Request
Please note the following:
- Certificates are only obtainable if the Sacrament was celebrated at St. Lawrence.
- Please submit a separate request for each Sacrament celebrated.
- Please allow 7-10 days for your request to be processed.
- The Archdiocese of Atlanta requires verification of the requester’s identity. Signed certificates may only be issued to the individual who received the Sacrament(s) or to their parent(s).
- Requests submitted directly from another parish are also permitted.
You will have the option to upload a copy of your photo ID below. For your security, ID images will be retained only for the time necessary to complete the request and will be deleted once the certificate has been picked up or mailed out. If you prefer, you may instead present your ID in person at the time of pickup.
