• Consent to Groom

    & General Liability Waiver
  • Because we care about the safety and wellbeing of all pets, we want to assure you that we will make every effort to make your pet's grooming visit as pleasant as possible.

    1. If during an appointment it becomes apparent that your pet is not able to be groomed due to aggression/behavior or any other issue, the customer will only be charged for what services have been performed. Minor injuries such as nicks from clippers, scissors, or toenail trimmers may result if your pet does not respond to the groomer and remain still during the grooming or toenail trimming procedures.
    2. If your pet has any allergies or medical conditions (epilepsy/seizures, diabetes,  arthritis, separation anxiety, asthma, etc.), it is extremely important that you make the groomer/PET DEPOT aware of any medical & behavioral issues so that we can take the necessary steps to ensure your pet's utmost safety and comfort.
    3. Pet guardian/Customer assumes all liabilities, financial and otherwise, for the behavior and health of their pet in both the professional groomer and self-wash areas. In consideration of the services rendered by PET DEPOT, Customer waives any and all claims, actions, or demands of any nature, foreseen or unforeseen relating to the care, control, and health of customer's pet arising during or after services performed by the Groomer/PET DEPOT.
    4. Expression of anal glands are done externally by PET DEPOT groomers. If you find your pet to have persistent irritation of their rear end, we encourage you to speak with our veterinary staff as your pet could be experiencing irritation from the external expression provided during their time in grooming or from a preexisting condition.
    5. Occasionally, grooming can aggravate a current, or expose a preexisting medical condition. Such medical conditions could arise during or after the grooming procedure. In the event of this situation, The pet Guardian/Customer shall not hold the Groomer/PET DEPOT liable. Pet guardian/Customer shall not hold the Groomer/PET DEPOT liable for any skin irritations that may result from grooming procedures or any stressful effects that grooming may have on young, geriatric, timid, or temperamental pets.
    6. Pet guardian/Customer also shall not hold the Groomer/PET DEPOT liable for any problems uncovered on a badly matted or otherwise neglected coat or integument. 

     

    PET DEPOT holds the highest standards to ensure that your pet is handled with respect and cared for properly. We recognize and respect that all pets are living beings who have feelings and experience emotions. We value that you have entrusted in us to provide our services to them.

  • Emergency Medical Consent

    In the event that an unforeseen medical emergency occurs with your pet during their time in grooming, during normal business hours, PET DEPOT Veterinary Group and its veterinarians have the ability to perform any medically necessary remedies required to stabilize and/or treat your pet. The initial treatments required to stabilize your pet’s condition, including but not limited to examination, intravenous catheterization and fluid administration, rapid diagnostic tests, and emergency medication, may range from $300 to $950 but can exceed this amount in very critical situations. PET DEPOT and its affiliates will make all reasonable attempts to contact you in the event that your pet must receive emergency medical treatment.

  • By signing below, I acknowledge all of the statements above and understand and agree to release PET DEPOT, its groomers, owners, and affiliates from any and all liabilities, expenses, and costs (including veterinary and legal fees) resulting from any service provided; or unintentional injury to my pet while under their care or afterward.

    I formerly grant groomers associated with PET DEPOT permission to groom my pet(s).
