Media Release
In consideration of the right to receive services from and/or participate in the activities (collectively, the “SERVICES”) of Family Connections, Inc., a New Jersey 501(c)3 non-profit corporation (“FAMILY CONNECTIONS”), the undersigned (the “UNDERSIGNED”) and/or the undersigned’s parent or legal guardian if the UNDERSIGNED is under the age of eighteen (18) years, hereby agree as follows:
1. The UNDERSIGNED hereby voluntarily grants to FAMILY CONNECTIONS (and its affiliated entities, partners, successors and assigns) permission (without compensation except as aforesaid) to take and to use, reproduce, publish, transmit, distribute and display photographs or video, in whole or in part, including without limitation voice and likeness of the UNDERSIGNED, as well as any art work made by the UNDERSIGNED in connection with the SERVICES (and copies thereof) (collectively, the “ART”), including without limitation all printed matter associated therewith (collectively, the “MEDIA”) for use in FAMILY CONNECTIONS’ internal records, and for promotional, training or educational materials, which records and materials include, without limitation, annual reports, catalogs, brochures, posters, magazines, media articles, CD-ROM, DVD, electronic and paper newsletters, and all electronic media, including FAMILY CONNECTIONS Internet web sites, blogs, Social Media pages, and other similar web-based media, as well as (as to the ART) publicly displaying same or copies thereof. The UNDERSIGNED agrees that FAMILY CONNECTIONS will own all MEDIA taken by it or its employees or agents; provided, however, that FAMILY CONNECTIONS will not own the originals of all ART. Notwithstanding the foregoing, FAMILY CONNECTIONS will not include the UNDERSIGNED’s last name, address or other identifying information in connection with the use of any of the MEDIA. For avoidance of doubt, the UNDERSIGNED’s first name may be used in the MEDIA.
2. The UNDERSIGNED further understands that the Internet is not a secure medium, that any material posted on the Internet is accessible to anyone with Internet access, that while FAMILY CONNECTIONS has made efforts to secure its web site, it cannot guarantee privacy or control access to its web site, and that any MEDIA of the UNDERSIGNED that are posted on the Internet may be viewed or downloaded by any computer.
3. By signing this release form the UNDERSIGNED agrees that the UNDERSIGNED does not have any right to: (i) inspect or approve the MEDIA of the UNDERSIGNED; (ii) claim entitlement to any payment or other compensation for FAMILY CONNECTIONS’ use of the MEDIA; or (iii) make any claims against FAMILY CONNECTIONS or its affiliated entities, partners, employees or agents based upon the use or discontinuance of use of any MEDIA (including without limitation any claims for libel, defamation or invasion of privacy).
4. The UNDERSIGNED has the right to withdraw the UNDERSIGNED’s consent at any time, by written notice thereof to FAMILY CONNECTIONS, which notice may be provided by:
a. email to: hope@familyconnections.org, and followed promptly by telephonic notice to 973-323-3412; or
b. hand delivery, United States postal service (certified mail, return receipt), or by reputable national overnight courier providing proof of delivery, addressed as follows:

Such notice shall be deemed effective upon receipt by FAMILY CONNECTIONS. Notwithstanding the foregoing, any such withdrawal shall not affect or limit FAMILY CONNECTIONS’ rights to continue using any MEDIA created prior to such withdrawal.
The UNDERSIGNED (and the UNDERSIGNED’s parent or legal guardian, if the UNDERSIGNED is under the age of eighteen (18)) has read this MEDIA RELEASE and understands its contents. BY SIGNING THIS MEDIA RELEASE, THE PARTICIPANT (AND THE PARTICIPANT’S PARENT OR LEGAL GUARDIAN, IF THE PARTICIPANT IS UNDER THE AGE OF EIGHTEEN (18)) ACKNOWLEDGES THAT HE/SHE HAS READ AND UNDERSTOOD ALL OF THE TERMS CONTAINED HEREIN AND THAT THE PARTICIPANT IS GIVING UP SUBSTANTIAL LEGAL RIGHTS.
