By signing and submitting this form you acknowledge that the Vista at Cedar Hills will take payment upon receipt of your request using your card on file. You will be charged for all decor/items brought to the event room whether the item is used or not. Your items specified in this form will be ready for you in the room upon your arrival at the Vista at Cedar Hills. You understand that upon your arrival, you take responsibility for any loss or damage to any Vista at Cedar Hills rental items and/or property. By signing you accept that if any rental items are returned damaged, broken, or not returned you will be charged to the credit/debit card on file to replace the items. You will be charged as early as the following morning for replacement items. Please note that the replacement fee is not the same as the rental fee.
