Vaccination and Spay/Neuter Promissory Contract

In consideration of Billy the Kidden Rescue (BTKR) releasing the above described kitten(s)/cat(s) to me for adoption, by clicking Yes, I agree to the following conditions of this adoption. Any No answers will require further discussion and may prevent me/my family from adopting from BTKR.
1. I understand I am financially responsible for all costs associated with veterinary care of this animal to include spay/neuter and rabies vaccine. (Unless otherwise noted on the Adoption Contract)*

2. I will stay in communication with BTKR to report that this/these kitten(s) have been vaccinated and spayed/neutered when kitten(s) weigh at least 5 lbs for females and 6 lbs for males.*

I will provide BTKR with a copy of the vaccination and spay/neuter record clearly stating the date, description of kitten(s), name of kitten(s), age of kitten(s), and treatments performed within 1 week of the date of the procedure. 5. I agree that BTKR has the right, at their discretion, to reclaim this animal if there is reason to believe the vaccinations and spay/neuter have not been done within the above weight requirements unless special arrangements have been made.*

6. I give permission for a representative of BTKR to come onto my premises where the animal(s) is kept for the purposes of verifying spay/neuter or retrieving him/her if these conditions have not been met.*
