  • This is a document where you choose someone who can make decisions for you once you can no longer make your own decisions.

  • This document allows you to say what type of medical services you want in case you become incapacitated, and designates someone who can made medical decisions for you.

  • This allows you to transfer your real estate to someone automatically upon your death (avoiding probate).

  • An adult guardianship is for a person who is at least 18 years old (or within 6 months of turning 18) who is disabled or incapacitated and is unable to meet essential needs or manage finances.

The application for the Right to Counsel program opens at 12:01 a.m. every Monday morning and closes once all the openings for the week have been filled.
