I agree to Voluntarily Transport live animals on behalf of said dog's sending/receiving individual(s), shelter(s) or rescue(s).
I have a legal, current driver's license and current liability insurance on the transport vehicle.
I have no interstate travel restrictions due to federal or state regulations. I am not wanted by any authorities.
I agree that if at any time I am unable to complete the transport, I will contact the sending/receiving individual(s), shelter(s) or rescue(s) and bring said animal(s) to the nearest animal shelter.
I understand Removing an animal from a transport before it has reached the final destination constitutes theft in all 50 states and will be reported to local, state, or federal law enforcement agencies.
I hereby agree to transport and care for said animal(s) in a safe and prudent manner to the best of my abilities.
I hereby agree to provide water to said animal(s) as needed.
I hereby agree to feed said animal(s) (should the transport extend 8 hours or more)
I hereby agree to allow said animal(s) bathroom/exercise breaks as needed. (expect every 2-3 hrs)
I hereby agree to keep said animal(s) on a leash and to secure collars and leashes before exiting the vehicle.
I herby release the sending/receiving individual(s), shelter(s) & rescue(s) from any liability for any incidents related to bites, injuries, property damage, or illness, that may occur during transport and/or overnight stays.
