    Please read carefully, complete and sign this form before your appointment.
  • MOST IMPORTANTLY: Don't attempt to remove extensions on your own, as this will result in permanent hair loss.

  • DO'S

    • Using suggested extension brush, brush your hair before shampooing . This will ensure it's tangle-free.
    • Secure the hair with one hand while detangling to avoid any pressure or tension on the extensions or natural hair.
    • Use suggested shampoo, conditioner, mask, and leave-in treatment to ensure the longevity of the hair extensions.
    • When shampooing your hair, work vertically, not horizontally nor in a circular motion. This will create fewer tangles and be easier to manage.
    • When applying conditioner, make sure to apply mid-lengths and ends, and avoid applying conditioner to the root area.
    • Always use a leave-in conditioner on the ends to keep them looking silky.
    • We recommend brushing your hair at least twice a day to keep your hair extensions from matting or tangling.
    • Weekly treatments are recommended.
    • Tie your hair into a loose side ponytail or braid when sleeping or during activities.
    • Avoid tight updos or ponytails for the first few days after hair extension application.
    • When swimming, always tie your hair into a ponytail or braid and apply a leave-in conditioner to the hair to act as a barrier against harmful chlorine, saltwater, sunblock, etc. Always shampoo immediately after swimming and detangle as normal.
    • Always cover your hair when in direct sunlight for long periods.
    • Protect your hair with a leave-in conditioner and/or heat protectant spray before using hot tools to protect the hair from damage.

    DO NOT

    • Don't go to bed with wet hair.
    • Don't massage your scalp when shampooing.
    • Don't apply oils, alcohol, or ethanol to the bond area.
    • Don't leave your hair wet for long periods of time without detangling, as matting may occur.
    • Don't allow bronzer, tanning oils, sunscreen, or products with a tint to contact your hair extensions, especially if you have blonde hair; these products may lead to a slight color change.
  • As an extension client of Clementine’s Salon and Skincare, I understand that the extensions purchased are non-refundable. Furthermore, I understand that the installation of extensions is a service provided by my stylist and that if I am dissatisfied with the installation of the extensions, I may receive a complimentary redo of the installation with my service provider; however, as stated above, I understand that the hairpiece itself will not be discounted or comped by Clementine’s Salon or its stylists. I have answered all questions honestly and understand the risk associated with extensions and the best way to care for my extensions.
