  • Integrated Health Hawaiiʻs (IHH) care coordination program bridges the gap between systems of care to improve timely access to social services, educational services, community resources, and medical and behavioral healthcare. IHH is conducting this survey to discover if there is a need for care coordination education.

    What you can expect if you participate:

    Participation is completely voluntary. You can stop participating at any time by closing your browser window or tab. This survey will take about 5-10 mins to complete. There is a total of 10 questions.

    Costs and benefits of participation:

    There is no cost for participating and there are no direct benefit to you for participating.

    Confidentiality:

    All answers for this form will be kept on an encrypted and password protected computer in a locked room. The will ask for identifiable information and an email address at the end of the survey so we may contact you in the future. If you want your answers to be completely anonymous, please do not fill out these questions. No identifiable information will ever be publically presented with this research.

    Compensation:

    There is no compensation for participating in this survey.

    Future research:
